Here is our TEDE ALARA Procedure and the forms we use for calculation.

Our forms are not particularly user friendly, so we’ve built an access database that allows us to perform them more efficiently while reducing human performance issues.

If you like, I can send you a copy of our database so you can try to reproduce it for your needs.
