[Global] Accumulator dumping

During the outage, we have had a first time issues with accumulator valve testing (Accumulator dumps into the cavity when it is full of water) resulting in airborne conditions in upper containment.

Does anyone else have this issue?

If so do you restrict access during accumulator dump activities in case of airborne conditions?
