Follow up - excerpt from post outage report 3EOC23:
Specific noteworthy items captured include:
* Steam generator activities were well executed despite higher than anticipated rad and contamination levels, completed work for 98% of estimated exposure with 0 personnel contaminations. Instrumental in this success was the development and use of down draft tables for the upper probe drivers.
* Co-ordination and execution of the initial drop & shock crud-burst went well despite challenges created by failed fuel and the late in cycle release of core corrosion products.
* The execution of the plan to mitigate the impact of failed fuel was noteworthy. The actions prevented any significant release of noble gases and no outage delays were recorded during initial system breaches. Highlights of the actions performed are:
* Maximized the effectiveness of our RCS demineralizers by replacement of resin.
* Conducted just-in-time training for operators on gaseous waste disposal (GWD) system header management.
* Installed charcoal filters in air filtering (HEPA) units for use during the outage and routed exhaust to containment purge inlets.
* Maintained reactor building purge in continuous mode early in the outage.
* Developed a detailed plan for managing personnel contamination event response.
* Increased RP monitoring and surveillance.

See if this gets you started. You know me - if it happened, I've got pictures.

Hi! Hope everyone is having a great summer.
We at DC Cook have a current fuel leaker in one of our units. We would like to perform a quick benchmark of other sites who may be dealing with something similar or have gone through an outage with one. Below are a few questions. Thank you for taking time to answer these questions.
Failed Fuel Benchmark questions:
1. Have you experienced an outage after you ran a cycle with a fuel leak?
2. What were your biggest take-aways from the outage?
3. Did you have a documented plan with actions?
4. Do you have a good RP contact that could assist with more questions?
1. Are you currently pre-paring for an outage with a fuel leaker?
2. Do you have a documented plan with actions?
3. What is your biggest concern?
4. Do you have a good RP contact that could assist with more questions?
Thanks,
Brian
