RP coordinator approves up to 10 mrem.
RP Head (Field Ops Manager) approves up to 300 mrem.
RP Director (RPM) approves greater than 300 mrem.

We don’t have an RP specific form, but there is a field on a Scope Change Request for ALARA to assign, and sign for, the dose estimate.

We are changing our work control process, and I wanted to include approval levels based upon Dose projections.  For example, ALARA tech, approves up to 5mrem, ALARA Supervisor Approves up to 20 mrem, RPM approves up to 50 mrem and ALARA Committee Chair approves greater than 50 mrem.  Do you have something similar in your work control process, if so can you please share your form and limits?
