At Davis Besse we use VSDS. We do not use any sort of survey approval template. Seems like a good idea, interested to see the responses.</div>

Air Sampling:</div>

We expect our technicians to list the <u>Air Sample Record Number</u> in the survey comments section. We do not require that the physical air sample record gets scanned into VSDS as an attachment (like I have seen other plants do.) Our air sample records go
to records as a stand alone document</div>
